Parent Consent for Health & Counseling Services
Under Senate Bill 12, Texas school districts must receive written parent or guardian consent before providing certain routine health and counseling services.
This includes some services provided by school nurses, athletic trainers, and school counselors. Certain situations, such as medical emergencies or suspected abuse or neglect, do not require prior parent consent.
To allow your child to receive services that require permission, please complete the consent form below.
Please remember:
- Complete a separate form for each student.
- Services requiring consent cannot be provided until the completed form is received.
